a movie set in Thailand (maybe) in early 1900's.?

a movie about a couple who live in some Asian country (like thailand,
i believe). I believe its set in the late 1800's or early 1900's. I
believe one of them maybe has an affair. I can picture there house on
a river (or a body of water), it has a huge deck with stairs heading
down to the water. He may have been a doctor, dealing w/some kind of
outbreak?
